Wiselife is a digital innovative platform dedicated to health literacy and to the management of chronic diseases, fully patient-centered. We offer health information, created by qualified professionals, in several format (text, infographic and video) with a constant focus on the patient, aiming to increase health literacy.We want to have personalised solutions to chronic disease management with online customized services, to accompany patients in managing their illness, optimizing the aspects of their life that enables them to live better with their condition. Wiselife provides the necessary information to improve quality of life and to guide patients’ choices in key moment of life. At Wiselife the patient can manage their health data, noting important aspects to discuss it with their physician.
